  2. A copy of your most recent peer/quality review, including the opinion letter, comments and your response thereto, and the TSCPA acceptance letter.
    1. Copies are not available (all files were seized by the Treasury and FBI)
    2. The peer review was completed in January 2005. The result were perfect No-Comments, TSCPA acceptance letter received about Feb 2005
  3. A description of the nature of your practice.  State the percentage of your engagements related to the following areas:
    1. Government audits: 0%
    2. Other audits: 0%
    3. Accounting compilations and reviews: 35%
    4. Tax: 60%
    5. Other (describe): Information System and IT Consulting: 5%
  4. A comprehensive list of your Continuing Professional Education classes within the last two years.  State the course title, the number of hours of credit for each class and the sponsoring institution.
